Also, you will have to complete the Medical Treatment Authorization form and bring one copy to the Extension Office with your payment and another copy with you to the camp for registration. Link to form below:

YES! Camp 2009
District 5 YES! Camp is set for November 13-14, 2009 at the Lakeview Methodist Conference Center in Palestine. All 4-Hers ages 8-13 are invited to attend. YES Camp will be lots of fun with great educational workshops, and those who attend always seem to have a great time! This is an excellent opportunity for junior and intermediate 4-H members in our district! The theme this year is Auto B Good. There will be a costume contest so have your kiddos dress up in some Automobile themed way, I.E. car logos, race car drivers, mechanics, pit crew, use your imagination; however, Please remember the 4-H dress code when dressing up!
The registration fee for YES! Camp is $40 per person (both youth and adults). A registration form is attached. Please contact Willie Arnwine in the CEO to get on the list. Payments can be made via cash or check (Make payable to: CHEROKEE 4-H COUNCIL). These funds are due in the CEO no later than 5 PM Tuesday, October 27.
Download, print and complete the medical release form. This form is to be completed by registered 4-H members and submitted at check-in at the Lakeview Methodist Conference Center. It will be returned to chaperons upon departure.
Attendees should plan to arrive between 7 pm and 8 pm on November 13 in costume and need to eat super prior to arriving. Saturday's departure time will be around 3:30 pm.
Campers should bring sleeping bags, billows, towels, toiletry items, flashlight, etc. to camp. Please look for a schedule and more information regarding the YES Camp theme in the coming weeks.

Each County needs to bring a box of junk that could be possible used to make a small race car. (bottles, tape rolls, bottle caps, etc.)
